内視鏡ドックのご案内住民健診や企業健診で調べている項目をご存知でしょうか。健診では高血圧や糖尿病、脂質異常症といった生活習慣病を中心に、貧血がないか、肝機能や腎機能に問題がないかを調べています。 健診が全ての病気を網羅して調べているわけではありません。 40代頃から気になってくるいわゆる「がん」に関しては、一般的な健診での発見は難しいでしょう。ですから住民健診などには、任意で「胃がん検診」や「大腸がん検診」が追加で受けることが可能になります。
「大腸がん検診」では一般的に便潜血検査を行います。便潜血検査が陽性の場合、大腸カメラ検査による精密検査が必要になります。しかし、便潜血検査で陰性であれば大腸がんでないというわけではありません。内視鏡ドックをお勧めする理由は胃がんも大腸がんも高い精度で発見することができるからです。40歳を超えたら一度、症状がない場合でも内視鏡検査を行うことをお勧めいたします。

自費診療による内視鏡とは国民皆保険制度による健康保険は、病気で治療が必要な患者の治療費を国民同士で支えあうためにあります。患者様には診療費の1－3割を窓口で負担していただき、残りの診療費は保険組合から医療機関に支払われる仕組みです。「家族が癌になって不安なので検査したい」や「今まで内視鏡検査をしたことがない」などの理由では保険診療による内視鏡検査はできません。病気ではない方（≒症状がない方）は自費による診療となります。
しかし、胃がんや大腸がんなどの病気は初期症状がないことも事実です。また、40歳以上から胃がんや大腸がんが多くなってくる事実を考えれば一度、ご自身のコンディションやがんのリスクなどを把握するためにも内視鏡検査はしておくべきと考えます。ただし、医師の問診上、内視鏡を行う理由により保険診療になることもございますのでご相談ください。

糖尿病とは､血液中に溶け込む糖分（血糖値）が常に高い状態になってしまう病気です。血糖は食事として食べた糖質が､腸から吸収され血液に溶け込み､身体を動かすエネルギーとして使われます。血糖をエネルギーとして使う際にはインスリンというホルモンが必要で､インスリンによって血糖は身体の細胞に取り込まれていきます。糖尿病はこのインスリンがうまく働かなくなった病気の事を指します。

インスリンは膵臓という臓器から分泌されるホルモンですが､そもそもインスリンが分泌されないタイプの糖尿病を1型糖尿病といい､インスリンの分泌量が少なくなったり､分泌されても身体が活用出来なくなっているタイプの糖尿病を2型糖尿病といいます。患者数が多いのは圧倒的に2型糖尿病であり､また一般的に生活習慣病として知られているのは2型糖尿病になります。

糖尿病の原因2型糖尿病はインスリンを身体が効率良く活用できなくなります。血糖値が高い状態が続くと､膵臓がインスリンを分泌して､血糖を下げようとします。しかし､ずっと血糖が高い状態が続くと膵臓がつかれてしまい､インスリンの分泌量が少なくなってしまいます。また､インスリンに反応して糖分を取り込む事が出来る筋肉や肝臓が､インスリンに対して鈍感になりインスリンの効きがわるくなります。これを「インスリン抵抗性」といいます。

これらの原因には、肥満、運動不足、武蔵野市 糖尿病 不健康な食生活、年齢､遺伝（体質）などが挙げられ､生活習慣により引き起こされる要因が大きい為､糖尿病が生活習慣病の代表として挙げられています。40才以上の方､太り過ぎの方､著しい運動不足､家族に2型糖尿病患者がいる方に発症しやすい事も分かっています。

それに対して1型糖尿病はインスリンを分泌する膵臓の組織が機能しなくなる病気です。生活習慣病でも、先天性の病気でもありませんし、遺伝する病気ではありません。子どもを中心に大人でもなってしまう病気で､インスリンが分泌できない為､それを補うインスリンの注射による治療が必要です。

糖尿病の症状2型糖尿病は、初期の段階では自覚症状が有りません。症状がでても長い期間を経てゆっくり進行し症状が出てきます。無症状では健康診断で糖尿病を指摘される事が多いですが､健康診断を受けていない方などは非常に重篤な状態になって初めて糖尿病に気がつく事も珍しくありません。

糖尿病の予防方法2型糖尿病は何より健康的な生活を維持する事です。標準体重を維持する事､定期的な運動を心がける事､バランスの良い食事をとって行く事が何より大切です。

糖尿病治療を始めるタイミングできれば糖尿病になる前に､気付き､予防する事が最も大切です。よく､患者様からは「糖尿病の気（け）があると言われたことがあります」というお話を伺います。では「糖尿病の気（け）」とはなんでしょうか。糖尿病の診断の一つに､空腹時の血糖値が126mg/dL以上というものが有ります。健常な方の空腹時血糖は70-110mg/dL程度で推移します。では､空腹時血糖が100-125mg/dLの方はどのように考えればいいでしょうか。これはインスリンの効きが悪くなり始めている可能性を示し､その結果としてすこし血糖値が高くなっており､要注意の状態と考えます。

また人は食事をすると糖分が吸収される為血糖値が上がりますが､正常な方は140mg/dL程度で血糖値の上昇は止まります。これは正常にインスリンが働き､血糖をあげないようにしているからです。糖尿病の方はインスリンが働きにくいため血糖値は200mg/dL以上になっていきます。しかし､やはり､インスリンの効きが悪くなると､食後血糖も170-200mg/dL程度まで上がってしまう事があります。

このように一般的に「糖尿病の気（け）がある」方は糖尿病の診断基準を満たしてはいないが､血糖が徐々に高くなりつつある人の事を指しています。医師にズバっと「糖尿病です」と言われた方は､前向きに治療に向き合って下さい。しかし､一番大切なのは糖尿病になる前､血糖値が高くなりつつあるとき（＝インスリンの効きが悪くなり始めているとき）に､まずご自身が自覚し健康的な生活を始める事が一番大切です。健康診断で､空腹時血糖が100-125mg/dLの方や食後血糖が170-200mg/dLの方は､治療として生活習慣を改める必要性があるかもしれない事を覚えておきましょう。

糖尿病治療を放置するとどうなるの?糖尿病で高血糖の状態がつづくとどうなるのでしょう。高血糖の状態がつづくと､身体では血管にダメージが蓄積していきます。高血糖は血管の内側を傷つけ､正常の機能を保てなくしてしまうのです。糖尿病になり持続的に高血糖状態が続くことで血管の劣化が原因となる病気をひきおこしやすくなります。糖尿病には3大合併症というものがあります。

一つは糖尿病性網膜症。これは眼の網膜にある非常に細い血管が傷つきます。進行してしまうと失明の原因になります。次に､糖尿病性腎症。これも腎臓にある非常に細い血管が傷付く事で起こる合併症です。身体の老廃物を尿として排泄する腎臓の機能が失われてしまうため、最終的に透析治療が必要になることがあります。

最後の糖尿病性神経障害は、高血糖により、足の神経に異常をきたし、足の先や裏、手の指に痛みやしびれなどの感覚異常があらわれる合併症です。つま先の感覚などがなくなりケガした事も気がつかない事も珍しく有りません。これらは細かい血管の異常が引き起こす合併症ですが､同時に太い血管がダメージを受けると血管が硬くなる動脈硬化を起こしやすくなり､脳卒中や心筋梗塞の重大な原因になります。
